Use lead-coated copper. Run top edge up well under shingles. Make sure that leading (overflow) edge is below shingle line above. Pitch from middle of roof to soldered drop tubes at each side (if you're good, you can do this with sheet metal within the gutter and keep the front edge level).

What Jeff said, except don't nail thru the fancy metal you just formed and soldered when you're installing the shingles. Cleat the back edge of the metal rather than nailing it down, so it can expand and contract. I normally install ice barrier membrane over the back edge of the metal before shingleing.
