I have to level a floor that IS 1/2″ TO 1 1/2″ HIGH ON ONE SIDE. Once it is leveled I am planning to finish it with hardwood flooring. The room is about 16X16 and it is on the 2nd floor. sheeted with 5/8″ plywood on 2X12 joists 12″OC. I have currently an 8′ ceiling.

how about using 2x4's cut on a taper and new new sheeting. with proper glueing and screws shouldn't have any trouble with squeaks. i would use 2x material just to have a little meat left. dont think that you would notice the slightly lower ceiling height.

Look into using Gypcrete.It is a self-leveling concrete liquid.With hard-wood floors you probably want to be pretty close to level.
I HEARD THAT THIS CONCRETE WOULD CRACK IN AREAS THAT IT IS TOO THIN. IS THIS TRUE?
Is the floor 1 1/2" high on one side or 1 1/2" low on the other? How old a building? The reason for asking is to determine what is causing the out of level condition. Having renovated a couple of older houses nothing is more frustrating than dealing with previous partial fixes. When you re-level a house the remodeled bathroom gets a slope and the tub doesn't drain. If the building has settled askew then fix that. Jack up, shore up or support what is causing the out of level condition. If this is new whacky construction then by all means shim or float.
